Clover Health and MedZed Partner to Provide Virtual Annual Wellness Exams

An increasing number of patients are staying home and avoiding doctors’ offices due to fear of exposure to Covid-19. As a result, a large percentage of the population living with complex chronic conditions have missed regularly scheduled annual assessments. This is particularly troublesome for patients with chronic diseases who rely on regular medical management to prevent an exacerbation of symptoms.

As part of our commitment to deliver care to these patients, we are excited to announce a new partnership between MedZed and Clover Health. We will provide home-based annual wellness exams to identify social and clinical needs led by a remote provider connected over our platform. The partnership augments Clover Health’s existing annual wellness visit program with a focus on targeting those members most hard to reach. Our unique model of using a community health navigator on the ground connected to one of our primary care providers over our telehealth platform allows us to conduct annual health assessments in a manner that is convenient for patients, affordable for health plans, and effective at identifying medical and social needs to get the patient connected to the right services.

MedZed’s experience utilizing telemedicine and delivering services to managed care populations creates a great foundation for this pilot with Clover. We are committed to providing access to care in a consumer-friendly model. Our community-based outreach teams have demonstrated success in finding and engaging over 50% of the hardest to reach members.
